Are you a qualified and experienced counsellor or social worker looking to work within care coordination.

An established community care provider require an experienced care coordinator to work within their intake and assessment team.

The ideal candidate will hold a bachelor degree in social work or counselling. Have working knowledge of case management within a wide range of client groups including child protection, Mental Health and Housing.  Understand the community services with regards to referrals and outcomes.
